Skilled Nursing

Home health nurses provide care to patients in their own homes, including monitoring their health, treating wounds, performing tests, and administering medication following a physicians plan of care. Our skilled nursing staff are trained professionals not only in their field but have a deep passion for helping our patients.

Certified Home Health Aides

Our home health aids are available to help with daily living activities such as bathing, dressing, and basic hygiene.

Speech Therapy

Our speech therapists are licensed and certified professionals who can help adults who have difficulties with communication, eating, drinking and swallowing.

Physical Therapy

If you have a problem that results in pain or loss of functional mobility, you may benefit from the skilled services of a physical therapist. Your therapy will be focused on helping you restore normal mobility so you can get back to the things you enjoy.Home-care physical therapy occurs when a physical therapist comes to your home to provide rehabilitation services. This type of service is reserved for patients who are unable to leave the house to attend PT.

Occupational Therapy

Our occupational therapists provide rehabilitative services to patients with mental, physical, or developmental impairments. Whether someone has trouble performing day-to-day activities due to an illness, injury, or disability, our occupational therapists teach our patients how to work within their limitations so they can live as independently as possible.

Arranging Services

Request for evaluations may be made by a physician, hospital, friend, or family member. Upon approval by the physician, a registered nurse or therapist will come to the home to evaluate the patient’s medical needs and coordinate the development of an individualized care plan. Our staff will record and submit the necessary paperwork. We work closely with physicians and family to ensure that any changes in the patient’s plan of care updates as required.
